SUMMARY:
Provides pediatric and adult medical care and treatment in a primary care clinic setting.
This position may have remote work requirements.
This position is subject to all current CMS vaccine mandates.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
ü Performs physical examinations and preventative health measures within prescribed guidelines and approved protocols
ü Orders, interprets, and evaluates diagnostic tests to identify and assess patient’s clinical problems and health care needs
ü Records physical findings, and formulates plan and prognosis, based on patient’s condition
ü Discusses case, as needed, with Medical Director and / or other health professionals to prepare comprehensive patient care plan
ü Submits health care plan and goals of individual patients for periodic review and evaluation by the Medical Director or his designee
ü Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures
ü Refers patients to specialists for consultation or to specialized health resources for treatment, as needed
ü Participates as instructor / facilitator for in-house employee education
ü Participates as instructor / facilitator in community education and outreach
ü Ensures all credentialing, continuing education requirements for continued licensure are current
ü Adheres to all current Primary Healthcare clinical protocols and family practice standards of care
ü Participates in all required Primary Healthcare Centers, HRSA, OSHA and Emergency Preparedness and other training
ü Adheres to all HIPAA Rules and Regulations
ü Treats staff, providers, visitors and patients with dignity and respect
ü Participates and helps maintain a Patient Centered Medical home environment
ü Other duties as assigned
SUPERVISORY RESPONSIBILITIES:
Provides supervision and oversight of support staff as indicated.
QUALIFICATIONS:

EDUCATION AND EXPERIENCE:
Advanced nursing degree (nurse practitioner) from an accredited college or university
Two years’ (minimum) experience in supervised medical practice preferred.
LANGUAGE ABILITIES:
Ability to read, analyze, and interpret general medical information. Ability to effectively present information and respond to questions about patient care.
MATHEMATICAL SKILLS:
Strong math skills required.
REASONING ABILITY:
Ability to apply common sense understanding to carry out instructions given either written or oral. Ability to do problem solving.
CERTIFICATES, LICENSES, REGISTRATIONS:
Licensed in the State of Georgia
Board Certification as appropriate
CPR Certification
PHYSICAL DEMANDS:

WORK ENVIRONMENT:
The noise level in the work environment is usually low.
