Summary/Objective This position is responsible for providing intensive case management services to the families, using a family-centered approach, and assists clients in obtaining goals that lead to stability and permanent housing, employment, and self-sufficiency. Provides structure, direction, and service linkage to connect families to available resources in the community and overcome barriers to housing placement.

PRIMARY DUTIES Completes an in-depth needs assessment and develops a detailed Individual Service Plan (ISP) for each family that includes specific goals, and action plan to obtain permanent housing and self-sufficiency, with a focus in the areas of:
Increasing income
Life skill development
Employment /Education
Physical and mental health
Fiscal management
Childcare and Parenting
Meet bi-weekly with each family to track progress in achieving goals and
fulfilling responsibilities detailed in the ISP and Case management Agreement, and provides direction and counseling as needed to assist in meeting goals.
Provides service linkages to families in the areas of health, mental health, education and employment, housing, mainstream benefits, and parenting assistance, and monitors attendance at on-site classes for life skills, parenting and employment readiness.
Keeps accurate and up-to-date case records and case notes on client’s progress.
Facilitate communication and collaboration between the shelters, schools, early childhood education agencies and families, and acts as a liaison between shelter staff, schools, families, and other service providers.
Work with parents to monitor the school attendance and performance of their child/children, and help parents respond to any educational challenges that arise.
Supervise children and youth during program hours to always ensure safety of young people, attending to crises when necessary.
Provide linkages to children and youth in the areas of education, health and mental health, and assist parents in identifying appropriate daycare resources for preschool children.
Ensure the enrollment of all age-appropriate children into summer camp and after school services.
Conducts parenting screenings for depression, intimate partner violence and substance abuse.
Identifies family needs and provides information and referrals to other community agencies on a regular basis.
Establishes and implements family support plans including goals, objectives, and activities.
SECONDARY DUTIES:
Provide leadership and supervision of Youth Workers and Volunteers to create structured activities during out of school time hours and ensure that all children are prepared for school daily and on time.
Keeps appropriate documentation of services/interventions, collects data as needed and submits to Director of Community Resource.
Documents services delivered, entering all data into the Baltimore County HMIS
Facilitates a Quarterly Family Support Group
Creates an atmosphere of trauma informed care and respect for Families.
Supports a welcoming and positive environment for volunteers and works with them effectively to achieve program goals.
Works cooperatively and effectively with Agency volunteers maximizing the volunteers’ experience while meeting the needs of the programs.
May work in outreach offices as designated.
May provide coverage for the front desk as designated.
Collaborates with the Shift Supervisor and Youth Worker to monitor participation in out of school time activities.
Keeps updated on the most effective means of accessing services available within the larger community and maintains an active data base for referrals and community partners.
Networks with the service community and coordinates with on-site service providers regarding support services offered to clients.
Collects monthly statistics and submits to Director monthly.
Other duties as assigned within the scope of the position.
Education Experience and Knowledge
An associate degree with major course work in Child services, social or behavioral sciences preferred.
2 years of data entry experience in a high pace environment.
Must possess a current Maryland driver’s license and access to a vehicle for business use.
May be required to use your own personal vehicle to fulfill your job-related responsibilities.
Bilingual preferred.
