Responsibilities
Conduct face to face visits with assigned clients
Identifies and resolves barriers that hinder effective care.
Ensures client is progressing towards desired outcomes by continuously monitoring care plan
Review and record patients medical histories
Keep records of patients progress, or lack thereof
Be able to relate with patients and their families
Make necessary patient appointments
Transportation of clients as needed
Tracking and Report mileage of client transports
Adhering to managers directives

Qualifications
Reliable vehicle and appropriate insurance for home visits.
Must have a Valid driver’s license and the ability to travel within the state, as required
Experience working in home settings with populations of diverse cultures and ethnicities preferred.
Able to communicate well verbally and in writing.
Comfortable with computers and experienced in writing detailed notes
Strong organization skills
