Assisting Hands (Hudson) is looking for an experienced RN with a valid license in New Jersey and a minimum of 1 year of Home Care experience.

Your responsibilities include:

Conducting thorough client assessments and re-assessments

Formulating comprehensive care plans

Performing 30-day check-in and 60-day care monitoring visits

Conducting supervisory visits

Regular communication with patients, families, caregivers and care-stakeholders.

Training Caregivers

Qualifications:

Valid Nursing license in State of New Jersey

At least 1 year experience in Home Care is preferred.

Has a valid driver's license and a reliable mode of transportation.

Able to travel to clients’ homes throughout Hudson and South Bergen county.

Strong communication and interpersonal skills
