    A well-established Home Care agency is seeking a Field Nurse Supervisor to join our team. This role is essential in overseeing the quality of care provided to our clients, ensuring compliance with regulations, and supporting our team of caregivers. The ideal candidate will have strong leadership skills, a passion for geriatric care, and a willingness to travel.

    Position Overview:

    As a Field Nurse Supervisor, you will be responsible for conducting client assessments, supervising home health aides, and ensuring high-quality care for our senior clients. You will collaborate with the care team to develop and implement care plans, monitor compliance, and provide ongoing training and support to caregivers. This full-time role requires travel to client homes and between offices, with a mileage reimbursement or travel stipend provided.

    Key Responsibilities:

    • Perform in-home client assessments and develop care plans tailored to individual needs.

    • Supervise and evaluate the performance of home health aides to ensure compliance with agency policies and state regulations.

    • Provide clinical oversight and support to caregivers, addressing concerns and ensuring proper care techniques.

    • Conduct routine visits to clients’ homes to monitor care quality and make necessary adjustments.

    • Educate clients and their families on care plans, medication management, and health maintenance.

    • Ensure adherence to state and federal regulations governing home care services.

    • Collaborate with the administrative team to maintain accurate records and documentation.

    • Participate in on-call rotation as required.

    Qualifications:

    • Active RN license in the state of New Jersey.

    • Geriatric care experience required.

    • Strong knowledge of home care regulations and best practices.

    • Excellent assessment, documentation, and communication skills.

    • Ability to travel regularly within Essex, Morris, and Union Counties.

    • Experience with Wellsky EMR is preferred but not required.

    • Proficiency in Microsoft Office and electronic medical records systems.

    • Reliable transportation.

    Compensation & Schedule:

    • Hourly Rate: $53 - $60 per hour

    • Full-time position

    • Mileage reimbursement or travel stipend provided
