    Summary of Position: Seeking a dedicated Float Nurse to provide direct patient care during evening and night shifts (7p-7a) with rotation every 4th weekend and holiday, including the EverHeart Care Center in Greenville, OH, US. We are in search of a skilled Licensed Practical Nurse (LPN) to join our expanding team at a medical facility in Greenville, OH, US. The ideal candidate must hold a valid state license and exhibit a proficient understanding of nursing practices and procedures. Responsibilities: Responsibilities of Position: Provide competent nursing care to assigned patients through skilled symptom management, emotional support, appropriate teaching, and timely response to patient and family needs, including: • RoutineVisits • End of Life/Continuous Care • Referrals • Care Center Assistance • Chart Reviews • Other duties as assigned. • Provide patient/family services according to the established Plan of Care and signed physician orders. • Recognize and report promptly to the registered nurse any changes in the mental and/or physical status of the patient. Reporting should be written and oral when appropriate. • Explain hospice services and benefits in regards to Medicare, Medicaid, and Private Insurances and knows when to use Palliative Care Interventions and ABNs. • Obtain and enter orders from Medical Directors and Physicians when deemed appropriate. • Order appropriate Durable Medical Equipment (DME) for the patient. • Facilitate continuity of care by coordinating admission of home-care patients to inpatient settings (hospital, nursing home), as needed. • Work evenings and weekends as needed to meet the needs of the patients, families and agency backup schedule • Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements. • Maintain clinical competency for assigned responsibilities and procedures. • Other duties as assigned. Qualifications: Education and Experience: • Graduation from an approved School of Nursing. • Current LPN licensure in the State of Ohio. • Minimum of two years of experience in Medical/Surgical, Oncology or community health nursing. • Hospice experience preferred. Special Skills and Requirements: • Shall have a valid driver’s license. • Excellent customer service skills. • Detail oriented with excellent communication, documentation, and organizational skills. Ability to follow instructions. • Ability to work independently without supervision. • Travel to other sites as needed. Travel may be in inclement weather. • Ability to lift 25 lbs of supplies or equipment. • Requires various amounts of sitting, standing, twisting, turning, bending, lifting, and walking. • Ability to assist with patient care activities such as ambulation, transfers, and positioning.
