    Northeast Center for Youth and Families, Inc. has a full-time opening for a Foster Care Clinical Case Manager (CCM) to be part of the Foster Care Services team in their Malden office. The Foster Care Clinical Case Manager is responsible for clinical care and management of client case load as assigned, specific to clients at high risk of psychiatric hospitalization or interventions, including duties such as weekly therapy, monitoring treatment and service planning processes, information flow among persons involved with clients, collateral partners, staff, foster parents and the like.

    The primary role of the Foster Care Clinical Case Manager is to provide intensive and clinical visits with clients specific to clinical contracts, provide regular mental health monitoring and provide timely crisis response. The Foster Care Clinical Case Manager will also provide risk assessments and emergency safety planning and create measurable clinical goals.

    The Foster Care Clinical Case Manager is responsible for holding weekly individual therapy sessions as well as family therapy sessions as indicated in treatment plan goals and initiate and monitor external recommendation processes and treatment planning for clients in case load.

    A Foster Care Clinical Case Manager must be capable of setting limits for youth, overseeing the development of interventions, and setting goals toward individual successes in conjunction with client’s guardians, therapist and other providers.

    An effective Foster Care Clinical Case Manager will interact in a timely and effective manner in reporting as directed for computer reports, tracking, paper reports and the like in accordance with agency protocols; will ensure compliance with policies and procedures, including all contract sources, and keep abreast of trends, significant events in the field of Human Services and Case Management.

    A successful Foster Care Clinical Case Manager possesses and exercises strong interpersonal skills throughout all levels of Agency, is able to analyze situations, identify problems, identify and evaluate alternative courses of action, and responsibly, accurately, and completely document all work and share as appropriate within FBS and with collateral partners.

    Other expectations of the Foster Care Clinical Case Manager are the ability to plan and organize work effectively and ensure its completion, use discretion and individual judgment with agency under the guidance and direction of supervisor, participate in on-call services as needed, and complete other duties as assigned related to these primary responsibilities.

    Qualifications include:

    Master’s Degree and Licensure eligible

    Ability to read, analyze and interpret client information, reports, clinical documents, education plans and the like.

    Ability to reason and solve practical problems, deal with variety of concrete variables and interpret instructions furnished in various formats.

    Strong interpersonal skills and ability to develop strong working relationships.

    Cultural competency across diverse populations.

    Experience working with at risk populations in clinical therapeutic settings.

    Candidates must have a valid driver's license and acceptable driving record; travel is involved, and a reliable vehicle is needed.

    Verification of adequate personal auto insurance on the vehicle(s) to be used, including, at minimum, Optional Bodily Injury to Others (coverage part 5 on a Mass Auto policy) limits of $100,000 per person/$300,000 per accident, and Damage to Someone Else's Property (part 4) limits of at least $100,000.

    Candidate must undergo EEC screening including CORI and DCF checks.
