    Job Overview:

    The Physical Therapist (PT) in a home health setting is responsible for evaluating, treating, and managing patients in their homes. The PT works closely with physicians, nurses, occupational therapists, and other healthcare providers to develop personalized care plans and ensure the patient's recovery and rehabilitation goals are met. This role requires excellent clinical skills, compassion, and independence.

    Key Responsibilities:

    Patient Assessment and Evaluation:

    Conduct thorough initial evaluations and reassessments of patients in their home environment.

    Perform physical examinations to assess strength, range of motion, balance, and functional mobility.

    Develop individualized treatment plans based on patient needs and goals.

    Evaluate home safety and provide recommendations for modifications if necessary.

    Treatment Planning and Execution:

    Administer therapeutic interventions to improve mobility, reduce pain, and promote functional independence.

    Educate patients and caregivers on exercises, posture, safety, and techniques to maximize function and prevent injury.

    Use evidence-based treatment modalities such as manual therapy, therapeutic exercises, modalities (heat, cold, electrical stimulation), and functional training.

    Collaboration and Coordination:

    Work collaboratively with interdisciplinary team members, including physicians, nurses, and occupational therapists, to coordinate patient care.

    Communicate regularly with the clinical supervisor regarding patient progress, updates, and changes in care plans.

    Ensure timely documentation of patient visits, treatment progress, and outcomes.

    Patient Education and Home Program Development:

    Provide education on injury prevention, mobility strategies, and chronic condition management.

    Develop and instruct patients and families on home exercise programs (HEP) to promote self-management of their condition.

    Documentation and Reporting:

    Complete accurate and timely documentation of patient evaluations, treatment plans, progress notes, and outcomes in compliance with company policies and regulatory requirements.

    Ensure all documentation adheres to Medicare, Medicaid, and insurance provider standards.

    Quality Improvement:

    Participate in quality improvement initiatives and in-service training to stay up-to-date with industry standards and best practices.

    Provide feedback to the leadership team regarding opportunities for improving patient care and operational efficiencies.

    Required Qualifications:

    Education:

    Bachelor’s, Master’s, or Doctorate degree in Physical Therapy from an accredited program.

    Licensure/Certification:

    Active Physical Therapist license in the state of practice.

    CPR/BLS certification.

    Experience:

    Minimum of 1-2 years of clinical experience, with a preference for prior home health or outpatient rehabilitation experience.

    Skills/Competencies:

    Strong clinical reasoning and patient assessment skills.

    Ability to work independently with minimal supervision.

    Strong communication and interpersonal skills to collaborate with patients, families, and team members.

    Excellent time management and organizational skills.

    Knowledge of home health regulations, including Medicare and Medicaid.

    Preferred Qualifications:

    Experience in home health physical therapy.

    Familiarity with home health care documentation systems and electronic health records (EHR).

    Specialization or experience in areas such as geriatrics, orthopedics, or neurological rehabilitation.

    Physical Requirements:

    Ability to travel within the designated geographic area to visit patients in their homes.

    Physical ability to assist patients with mobility and perform therapeutic exercises.

    Occasional lifting and moving of equipment or patients, if necessary.

    Working Conditions:

    Full-time, with a flexible schedule to accommodate patient needs.

    Travel to patient homes within a defined region.

    The role requires a high degree of self-motivation and responsibility for managing patient caseloads efficiently.
