    Job description- Full Time Physical Therapist

    The Physical Therapist evaluates, plans and implements a goal oriented Physical Therapy treatment program that focuses on restoring function and preventing further disability as a result of orthopedic conditions, neuromuscular disease, trauma, amputation, congenital defects, surgery and other debilitating illness; provided in the safety and comfort of the patient's home, with interdisciplinary collaboration with Occupational and Speech Therapy.

    Responsibilities:

    A day in the life of a Physical Therapist at Pearl Rehabilitation LLC

    1. Team Member: Participates in the interdisciplinary rehab model to create thorough plans of care for patients and families

    2. Customer Service: Delivers the highest level of customer service to all patients and their families

    3. Delivery of Care: Models and maintains a safe environment in which to deliver services and implements appropriate treatment techniques and skills

    4. Effective Communication: Develops a rapport and mutual respect with the patient and families to foster an optimum environment for goal achievement. Provides education to patients and families based upon the identified learning needs, in a manner that the patients and families can demonstrate their understanding.

    5. Skilled Evaluation: Gathers all information necessary to properly evaluate the patient's needs, including appropriate tests and measures; as well as, consultations with other health care professionals, as appropriate.

    6. Plan of Care: Develops and implements an individualized treatment program based on findings during evaluation and establishes goals with patients and their families

    7. Treatment: Develops and implements an individualized program of treatment focused on identified patient needs

    8. Goal Setting: Formulates functionally based and objective short and long term goals

    9. Patient/Caregiver Education: Provides and documents education to patients and caregivers based upon identified learning needs in an understandable manner.

    10. DME Assessment: Demonstrates appropriate knowledge in assessing the need for DME for patients to improve their functional independence and safety. Follows through with appropriate professionals to ensure proper fitting of the DME to the patient. Educates the patient and family about the proper use and care of the DME

    11. Prosthetic/Orthotic Devices: Demonstrates appropriate judgment in ordering and measuring the patient for a prosthetic/orthotic devices. Assesses the fit and function of the device and the need for adjustments to ensure proper utilization. Develops with the patient and family or other caregivers, as appropriate, a wearing schedule to begin functional use. Educates the patient/family about the proper fit and function of the device and signs of malfunction which would indicate the need for intervention.

    12. Documentation Compliance: Prepares written documentation such as Daily Notes, Progress Notes, Reassessments and Discharge Summaries. Documentation completed as per departmental policy and procedure, and monitor the patient's status and progress towards goal achievement. All documentation and billing is thorough, timely, and compliant with all insurance guidelines and legalities

    13. Respect of Patient's needs: Administers evaluations and treatments in a manner appropriate to the patient's disability, demonstrating a significant respect for the patient's individulied needs

    14. Adheres to Pearl Rehabilitation LLC Organizational standards of behavior

    Qualifications:

    Education, Knowledge, Skills and Abilities Required:

    1. Graduate of an APTA accredited program or equivalent
    2. Certified in AHA Basic Health Care Life Support
    3. Physically able to perform tasks that may involve different body positions with proper body mechanics to prevent injury (ie: kneeling, squatting, twisting)
    4. Committed to providing exemplary care at home, coupled with compassion for helping patients achieve their goals, and excited ab out teamwork with one of the best interdisciplinary companies in Monmouth and Ocean County
    5. Physically able to perform tasks that incorporate pushing, pulling, lifting and/or carrying heavy objects (20 lbs. or more) without aid of physical devices
    6. Physically able to react/respond quickly in emergencies, including life threatening situations
    7. Excellent written and verbal communication skills
    8. Proficient computer skills that may include but are not limited to Microsoft Office, Google Suite and EMR platforms

    Licenses and Certifications Required:

    1. Physical Therapist License.
    2. AHA Basic Health Care Life Support HCP Certification
