Our Geriatric Resources team is made up of a group of kind, compassionate servants of the senior citizen population in the greater Northwest Phoenix area. We are committed to ensuring that the elderly who are entrusted to our care receive the best possible access to care & safety while honoring their dignity and autonomy. This important position allows us to help clients and families alike to navigate the aging process by providing strong education, support, and advocacy.

Our business is growing, and we are seeking new team members to join our fabulous Care Management team. Qualified candidates will have: At least 2 years of health care experience with the senior population; a curiosity for problem-solving; be impeccable with their word; have strong time management skills, communicate with compassion and confidence; exhibit professionalism and a commitment to supporting all members of our team.
Aging Life Care Professionals® offer a holistic, client-centered approach to caring for older adults or others facing ongoing health challenges. Working with families, the expertise of Aging Life Care Professionals® provides the answers at a time of uncertainty. Their guidance leads families to the actions and decisions that ensure quality care and an optimal life for those they love, thus reducing worry, stress and time away from work for family caregivers through:
Assessment and monitoring
Planning and problem-solving
Education and Advocacy
Family caregiver coaching
Qualified candidates must also be eligible for a path to National Certification through the National Academy of Certified Care Managers (NACCM), which includes (at minimum):
· Associates, Bachelor’s Masters or PH. D degree and is primarily engaged in a paid position in direct practice, administration or supervision of client-centered services to the elderly OR
· Associate’s Nursing degree or Diploma in Nursing and is primarily engaged in a paid position in direct practice, administration, or supervision of client-centered services to the elderly OR
· A person who holds an LPN, PTA or OTA license and is primarily engaged in a paid position in direct practice, administration or supervision of client-centered services to the elderly and their families.
· Must have/maintain Automobile Insurance Coverage 100/300/100 and list Geriatric Resources, LLC as an Additional Interest on the policy.
· Have a clear National Criminal Background Check upon hire.
· Must remain certified in CPR & First Aid.
· Must be eligible for work in Arizona through E-Verify screening
Flexible hours, on-call responsibility one week monthly, the ability to transport clients and the ability to maintain vehicle insurance standards are a must.
