    Job Summary The PACE Director of Clinical Services is responsible for overseeing and managing the clinical operations of the PACE program, ensuring the delivery of high-quality, participant-centered care. This role involves leading the clinical team, developing and implementing clinical policies and procedures, and ensuring compliance with regulatory standards. Reporting to the Program Director, the PACE Director of Clinical Services is responsible for overseeing the Clinical, Social Work, and Rehab services departments and is responsible for providing clinical guidance and support to the Interdisciplinary Team (IDT) as a member of the leadership team.

    Responsibilities · Accountable for the delivery of high-quality High Desert PACE clinic, social work, and behavioral health services. · Provide leadership, direction, and supervision to the clinical team, including physicians, nurses, therapists, and other healthcare professionals. · Provide ongoing clinical guidance and support which includes help in assessment, planning, implementation, and evaluation of participants. · Provide backup planning and facilitation of the IDT settings, as needed. · Direct clinic and social work including hiring, training, supervising, and mentoring team members and ensuring clear and effective workflows. · Ensure High Desert PACE participants experience smooth care transitions between settings (hospitals, skilled nursing facilities, home, etc.), deploy team members as needed, and work with the Home Care Team, Program Director, and Medical Director · Achieve key performance indicators as outlined in monthly operating reviewing and weekly tactical meetings. · Maintain compliance with PACE, ADHC, and DHCS regulations inclusive of audit prep, monitoring non-compliance, and implementation of corrective action plans. · Designing and implementing clinical operations strategies to help the organization achieve participant satisfaction. · Organizing and running all patient care or administrative operations and programs · Budgeting for clinical operations, including payroll. · Setting the tone and direction for clinical care · Working closely with other members of the executive team · Acting as a connection between participants, their families, and additional care staff. · Active, unrestricted California Registered Nurse (RN) License required.

    · Graduate of an Accredited School of Nursing; Bachelor of Science (BSN) preferred.

    · Master's Degree in relevant field preferred

    · Experience within the last five (5) years in an outpatient care clinic or health facility.

    · Knowledge and experience to treat participants and oversee complex clinical needs.

    Skills & Abilities · Thorough knowledge of current concepts and practices of health and social services for the elderly, particularly in home and community-based services and interdisciplinary team models · Working knowledge of health care delivery and financing systems, including Medicaid, Medicare, waiver programs, prospective payment systems, and the HMO/managed care programs · Must be able to maintain flexible work schedule in accordance with participant and program needs. · Excellent communication skills – interpersonal, verbal, presentation and written (also with ability to meet required standards of documentation). · Proficiency with Microsoft Office, including Word, Excel and PowerPoint Proficiency with Electronic Health Records · Ability to market and deal tactfully with prospective participants and the community. · Strong leadership and management skills with the ability to inspire and motivate a clinical team

    Education and Experience: · Master’s degree in Nursing, Healthcare Administration, or a related field, required. · Minimum of 5 years of clinical experience in a healthcare setting, with at least 3 years in a leadership or management role · Experience in geriatrics, long-term care, or a similar field is highly desirable · Current CA RN license, required.

    · Certification in geriatric nursing or a related specialty, preferred. · Work is performed in a healthcare environment with exposure to various clinical settings. · Current BLS and CPR, required

    Work Environment: · Work is performed in a healthcare environment with exposure to various clinical settings. · Regular interaction with participants, families, staff, and external stakeholders.

    · Requires adherence to infection control and safety protocols.

    Salary Range: $120,000 - $278,000
