    Housing Tenancy and Sustaining Services provides independent living skill identification, support, and development. HTSS overall goals are to identify current objectives, providing education and development of skills to assist the individual in an outcome of independence with a possibility to obtain independent living maintaining safe and stable tenancy once housing is secured, by offering the following services:

    POSITION: HTTSS Specialist

    ESSENTIAL FUNCTIONS:

    · Meets with each individual on caseload at least 3 hours a week.

    · Recognizes necessary intervention for behaviors that may jeopardize housing.

    · Presents education and training on the rights, and responsibilities of the tenant and landlord.

    · Preforms coaching, modeling, and promoting for evolving and sustaining important relationships with landlords/property managers/roommates with a goal of nurturing successful tenancy.

    · Communicates with the landlord, support coordinator, supervisor, case management provider, and family to address identified issues that could impact housing stability.

    · Assist in resolution of disputes with roommates, landlords and/or neighbors to reduce the risk of eviction or other adverse action including creating a repayment plan or identifying funding in situations in which the individual owes back rent or payment for damage to the unit.

    · Advocate and link with community resources to prevent eviction when housing is or may potentially become jeopardized.

    · Assist with benefits advocacy, including assistance with obtaining identification and documentation for SSI eligibility, medical assistance, and supporting the SSI application process.

    · Assistance with the annual housing recertification process.

    · Coordinate with the tenant to review, update, and modify their housing support and crisis plan on a regular basis to reflect current needs and address existing or recurring housing retention barriers.

    · Continue to assist with lease compliance, including ongoing support with activities related to household management, such as cleanliness, maintenance, and upkeep.

    · Perform health and safety visits, including unit habitability inspections.

    · Transports residents using your own vehicle to various destinations as directed.

    · Provide other prevention and early intervention services identified in the crisis plan that are activated when housing is jeopardized.

    · Provide education and guidance for independent living and life skills. Including but not limited to budgeting, financial literacy, social and communication skills, fire, kitchen, and public safety. As well as connection to community resources.

    · Attends assigned training or programs.

    · Attends staff meetings and provides input and ideas.

    · Attends quarterly and annual meetings with residents.

    · Other duties as assigned.

    REQUIREMENTS:

    · Awarded high school diploma (associate degree or higher in a related field preferred)

    · Have 1 or more years’ experience working with individuals diagnosed with IDD and or Autism

    · Have 1 or more years’ experience in providing services such as direct support professional, education, health needs, and counseling.

    · Ability to reach out to local agencies and resources as needed.

    · Have the capability and experience in managing complex caseloads while working with participants.

    · Experience/understanding the use of Microsoft applications such as SharePoint, Excel, and PowerPoint. Must have the ability to learn other computer software programs as required by assigned tasks

    · Understand the unique challenges and trauma that each participant might be processing and can provide the expertise and resources to help overcome those challenges as they arise.

    · Ability to follow all personnel policies and rules of the organization.

    · Ability to review all client accounts and financial records.

    · Ability to review and understand ISP’s while noting needed updates.

    · Ability to maintain and review records of client activities.

    · Ability to maintain confidentiality.

    LICENSE/CERTIFICATION NEEDED:

    · Ability to satisfactorily complete all necessary in-service training.

    · Possession of a valid driver's license and a demonstrated safe driving record
