Long term contract opportunity (1 year) for the nation's leading competitive electricity energy provider. The Health Care Occupational & Case Manager is responsible for coordinating and managing the care of patients with complex medical conditions or disabilities. Will work closely with patients, healthcare providers, and insurance companies to ensure that patients receive the appropriate care and support to improve their health outcomes.

Pay: $43.00 per hour

Location: HYBRID. Remote two days, Office three days (123 Energy Ave, \#765 Rockford, IL 61109)

Hours: M/F remote or in-office; Tues-Thurs: in-office: 6:45am-3:40pm; on-call required once every 7 weeks

Contract Dates: 12/2/24 - 12/1/25

REQUIREMENTS

• Assessing patients' medical, psychological, and social needs.
• Developing individualized care plans in collaboration with healthcare providers and patients.
• Coordinating healthcare services, including medical treatments, therapies, and consultations.
• Monitoring patients' progress and adjusting care plans as needed.
• Communicating with healthcare providers, insurance companies, and other stakeholders to ensure coordinated care.
• Documenting patient assessments, care plans, and interactions.
  

QUALIFICATIONS

• Must be a RN/PA or NP and be license in IL
• 2 years+ experience in occupational healthcare
• Case management experience (average caseload 30+ cases.)
• Telephonic case management for short term disability experience
• Background in nursing, social work, or a related field, along with experience in case management.
• Strong communication, organizational, and problem-solving skills. A compassionate and empathetic approach to patient care is essential.
• Basically Computer/Cellphone skills
