EMPOWER OTHERS TO IMPROVE THEIR HEALTH
As a Health Coach, you will communicate with individuals via the phone and digitally to assist members with any health-related concerns. The primary focus is to educate and guide members through their health care choices in order to be a more active and informed patient in their healthcare.
WHAT DOES A HEALTH COACH DO?
- Educate and guide individuals through options for managing their health
- Offer symptom support and answers to urgent questions
- Support individuals managing chronic and/or acute medical conditions
- Assist individuals in developing action plans to manage their health
- Help identify and overcome barriers to achieving health goals
- Provide evidence-based health education and resources
- Maintain a positive, professional relationship with patients and team members
ARE YOU QUALIFIED FOR A HEALTH COACH ROLE?
- Graduate of an accredited professional school or college/university
- 2 or more years working as a Registered Nurse in clinical setting
- Direct patient care experience in multiple nursing areas such as med/surg, home health, hospice, ER, critical care, etc. preferred
- Experience providing telephonic triage to patients in a setting such as ER, hospice or afterhours, preferred
- Experience supporting patients with chronic conditions, preferred
- Sound clinical judgment
- Technology savvy; experience using Windows-based software including the Microsoft Office suite
- Required to reside in a state that is part of the Nurse Licensure Compact and hold multistate privileges
- Active Registered Nursing (RN) license in good standing with multi-state (compact) nursing licenses, an ability to obtain other state licenses as dictated by business needs
- Preferred nurses that already have a CA and/or MN nurse license in addition to their compact license
- Excellent verbal and written skills; fluent, English/Spanish skills preferred
Job Locations
- This is a remote work from home position.
- Candidates are required to live and work in a state that is part of the nurse compact and hold a nurse license with multi-state (compact) privileges in their state of residence.
- It is preferred that candidates already hold a Minnesota (MN), California (CA) and/or Pennsylvania (PA) Registered Nurse license in addition to their compact license.
Position Type
- Full-time (40 hours per week)
- Two shifts available: 11:00 AM -7:30 PM Eastern Time or 5:30 PM – 2:00 AM Eastern Time.
- Shifts can be worked as four 10-hour shifts 11:00 AM – 9:30 PM ET or 3:30 PM – 2:00 AM ET.
- Candidates must be able to work one of the shifts listed above
