    Job Title: Health and Family Services Coordinator Reports To: Operations Manager FLSA Status: Non-Exempt

    Summary: The Health & Family Services Coordinator meets with community members in scheduled appointments and open office hours to coordinate referrals to local resources and services. Health and family services, include and are not limited to referrals to health-focused programs and services, supporting access to family supportive resources, and navigating applications for healthcare, housing, education, and other basic needs. The coordinator connects community members to vital health, family, and social services, fosters relationships with partner organizations, ensures efficient service delivery, collects quantitative and qualitative data, generates reports and gathers feedback to continuously enhance program quality and community impact.

    Position Duties and Responsibilities include the following. Other duties may be assigned.

    Participate in community, partnership, and organization meetings regarding health and family services, supervision, case consultations, and training.

    Complete intakes, assessments, case management, and required client file documentation in a timely, accurate, and legible manner.

    Coordinate client referrals to health and family services with community partners.

    Assist community members with applications for early, primary and/or secondary education, health care and/or other public benefits.

    Monitor the success of referrals provided and outcomes to ensure community needs are met.

    Organizational Duties and Responsibilities include the following.

    Answer incoming phone calls and inquiries, and refer to the appropriate department and/or staff as needed.

    Assists community members with LTCC services in the Resource Center.

    Ensures all services follow the guidelines of LTCC policies and procedures.

    Foster relations with community members and partner organizations to build community support.

    Maintains accurate records, files, and data systems for data reporting (e.g., visitor consultations, outputs, outcomes, evaluations, etc.)

    Compile data and narrative descriptions for monthly, quarterly, semi-annual, and annual reports.

    Complete training as required by the organization, program demands, funding streams, and individual development.

    Participate in public relations activities, outreaches, community events, and presentations.

    Participate in fundraising and LTCC community events.

    Assist with scheduling appointments and consultations for resource navigation.

    Other duties as assigned.

    Language Ability:

    Ability to read and comprehend instructions, short correspondence, and memos. Ability to write correspondence and consultation summary notes. Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees/leadership of the organization.

    Math Ability:

    Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret graphs and data sheets.

    Reasoning Ability:

    Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions. Ability to deal with problems involving a few concrete variables in standardized situations.

    Computer Skills:

    To perform this job successfully, an individual should have knowledge and expertise in Google Docs and Google Sheets.

    Education/Experience:

    High school diploma or general education degree (GED); or one to two years related experience and/or training; or equivalent combination of education and experience.
