    Doors to Freedom in Summerville, SC, is seeking a part-time compassionate, caring LPN or RN with experience in Pediatrics and Psychiatry preferred to assist the Director of Healthcare Coordination. In this role, the Healthcare Coordinator will provide advocacy at medical appointments, coordinate medical, dental, and specialty care, provide trauma-focused intervention in emergency situations, and be able to identify changes in behaviors and conditions. Advocacy is a key part of this position. Will provide medication administration, initiate new medications based on medical provider recommendations, and provide feedback on efficacy. Must be flexible to changes in schedules and plans. Must develop rapport easily with teens and have an overall interest in working with this age group and those who have experienced trauma. Good written and verbal communication skills are essential. Offering a competitive salary of $25 to $30 hourly. Responsibilities: • Administers medication to residents during shifts. • Reviews the Medication Administration Binder to ensure proper documentation. • With direction from the Director of Healthcare Coordination, oversees the healthcare coordination of services for residents. • Assists the Director of Healthcare Coordination in the intake process, file auditing, and discharge process. • Assists the Director of Healthcare Coordination with the completion of Weekly Staffings in order to update the staff, monitor residents’ progress, and ensure accountability and progress in the Individualized Growth Plan Evaluation - [PRG07-1]. • Engages with residents throughout the day. • Provides support to ensure residents’ rights, safety, and permanency needs are being met. • With direction from the Director of Healthcare Coordination, completes coordination and follow through of wellbeing needs for residents through required medical appointments and specialty appointments (as needed) and communicates these needs to staff, as designated by the Director of Healthcare Coordination. • Assists the Director of Healthcare Coordination with the completion and follow-through of the Medication Inventory [HC01], Prescription Medication [HC04], Standing Order [HC06], and Medication Refill Tracker [HC10]. • Transports residents to and from appointments as needed (mileage reimbursement will apply). • Maintains sensitivity to residents’ socioeconomic and cultural backgrounds. • Participates in Doors to Freedom’s Inclement Weather and Disaster Plan and follows assignments provided by the Executive Director or Healthcare Coordinator. • Fulfills other duties as assigned. Qualifications: • Strongly Preferred: LPN or RN with two years experience in management or supervision of child care personnel, a child care program, or a related field required. Compensation: $25 - $30 hourly
