We are looking for a Case Manager to enhance the quality of patient management, maximize satisfaction and promote cost-effectiveness. The successful candidate will be accountable for the full patient care cycle. Case Manager duties include assessing, planning, implementing, monitoring, and evaluating actions required to meet the client's health and human services needed. A certified case manager is a plus.

Qualifications
•Master's Degree in social work, counseling, or related fields with no less than 2 years of professional experience providing case management services, including intensive case management for complex cases, or bachelor's degree in a related field and 3 years experience supervising case management services
•Bilingual (English/Spanish) communication skills are required
•Ability to communicate clearly and effectively via oral and written means
•Ability to present a cheerful demeanor to individuals, their families, staff, supportive services personnel, and the public
•Ability to remain calm during difficult circumstances and emergencies
•Able to effectively manage significant documentation
•Advanced knowledge of case management services and community resources; Federal, state, tribal, and territorial disaster assistance programs, housing & utility assistance, and disability benefits
•Knowledge of the ORR policies and procedures as well as the corresponding state requirements services are being provided in
•Proficient in utilizing Outlook, Word, and Excel; PowerPoint, Access, Visio
•The ability to successfully pass a drug screen and a federal background check is mandatory
•Ability to deploy to all sites as needed
•OCONUS travel may be required (U.S. Territories only)
•Applicants selected will be subject to a U.S. Government security investigation and must meet eligibility requirements for access to classified information
Responsibilities
•Responsible for the frontline supervision and management of deployed Human Services Case Management lead case managers, case managers, and other deployed Human Services Case Management contract support staff
•This is on-site position travel will be required up to 75% if not local to area.**
•Ensures that deployed contract staff are successfully completing the safe and timely release of Unaccompanied Minors
•Ensures appropriate handling of all data collected by Human Services Case Management case managers
•Deploys when required to lead and support all active sites as needed
•Works flexible hours to lead and support influx, remote and emergency sites
•Responsible for staffing any complex cases requiring transfers, home studies, post-release services with FFS, CFS, lead case manager, and case managers
•Audits and analyzes data to ensure accuracy with the submission of Family Reunification Packets
•Collaborates with all shelter departments to ensure compliance with ORR requirements
•Responsible for tracking reunifications, discharges, transfers, and high-length-of-stay cases
•Provides OJT to the Case Management Department
•Provides continuous training to all employees, in accordance with policies and procedures, ORR updates, and Field Guidance
•Serves as a member of the quality assurance team
•Provides training to new case managers in the reunification, transfer, repatriation, and transfer process
•Prepares briefs and presentations as required utilizing the Microsoft Office family of products (i.e
Outlook, PowerPoint, Word, Excel, and Access)
•Supports meetings and teleconferences as required with management
•Performs other job-related duties as assigned
•Work will be performed in a variety of settings, including, climate-controlled office settings and outdoors in all types of weather
•Travel may be required up to 75% of the time
