What You'll Be Doing

• Leading in-home assessments and crafting custom care plans

• Guiding patients and families through treatment and medication

• Collaborating with providers and care teams to ensure seamless care

• Using your OASIS know-how to accurately document patient progress

What You Bring to the Table

• Current AZ RN license + CPR/BLS cert (or willing to get one)

• Home health experience with OASIS documentation

• Valid driver's license, reliable ride, and clear background

• TB test & fingerprint card (we’ll help if needed)

• Spanish-speaking? Even better!

• Nursing degree from an accredited RN program
