West Springfield, new 24/7, Open shifts, 7:00am - 3:00pm and 3:00pm - 11:00pm. We are looking for a Home Care Aide to join our team! You will be directly working with a female patient, following a one-on-one care plan in clients' home, this client is getting discharged from a rehab facility next week, desperately wants to come home, she's not allowed to go home until there is 24/7 assistance in place,

RESPONSIBILITIES

Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles

Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care

Maintain records of patient care, condition, progress, or problems to report and discuss observations with supervisor

Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert

QUALIFICATIONS

Personal care Experience

At least 2 professional work history references

One year prior eldercare experience

Driver’s license, Reliable transportation, and clean motor vehicle record

Willing to submit to criminal background checks

Willing to accept a shift every other weekend

Strong ability to read, write, and speak English
