Job Summary
We are seeking a Home Care Case Manager As a Case Manager, you will play a crucial role in ensuring the highest level of care and service to our clients. The ideal candidate is empathetic, highly organized, and possesses excellent customer service skills.
Responsibilities
Maintaining a fully staffed caseload at all times
Meeting all client needs and ensuring caregivers are appropriately matched and trained
Updating your supervisor on client birthdays within your caseload so a gift can be arranged
Ensuring client compliance with care plans, care assessments, and all necessary reports
Conducting 24-hour check-in calls following the start of service
Conducting a 10-day courtesy call with case managers after service initiation
Completing 30, 60, and 90-day reports for your clients
Performing bi-monthly home visits for quality assurance
Leading the initial intake process for new clients assigned to you
Ensuring care plans are updated annually for each client
Qualifications
High school diploma or GED
Previous experience as a Home Care Case Manager or in a similar position is preferred
Highly organized with attention to detail
Comfortable with Microsoft Office and other computer programs
Ability to multitask and prioritize projects
Excellent customer service and interpersonal skills
