Responsibilities: • Develop and implement home care plans for clients • Monitor client progress and make necessary adjustments to care plans • Coordinate with other healthcare professionals, such as doctors, nurses, physical therapists, and social workers, to ensure clients receive the best possible care • Maintain accurate and up-to-date records of client care plans and progress • Provide support and guidance to family members and caregivers • Ensure compliance with all applicable regulations and laws • Provide emotional support to clients and their families Qualifications: • A minimum of two years’ experience in home care or related field • Strong organizational, communication, and problem-solving skills • Able to work independently and multitask • Able to build and maintain strong relationships with clients and families • Able to work flexible hours Compensation: $60,000 at plan earnings
