About the Home Health Admissions Specialist position:

Reports to: Support Operations Manager

Assist in the coordination of patient care. ****

Home Health Admissions Specialist responsibilities:

1. Answers phones, receives all intake information for paraprofessional cases and enter the appropriate information in the computer.
2. Complete intake forms and route them appropriately for admission approval.
3. Schedule replacement workers as needed and input scheduling information.
4. Assist in communication with patients/families on home health aide related issues.
5. Handle matters related to staff and the patient they serve, with the exception of issues that require professional/clinical judgment.
6. Assist in the maintenance of case management notes related to personal problems and/other situations that arise with staff including those relating to patient care.

Home Health Admissions Specialist Qualifications

1. Minimum of two (2) years' experience, at least one (1) of which is in public health or home care nursing.
2. Demonstrates an ability to work with other health care clinicians and knowledge of third party reimbursement.
3. Self-directed and able to work with minimal supervision.
4. Demonstrate excellent observation verbal and written communication skills.
