Our busy home health agency is seeking an experienced home health aide to assist our case managers in implementing patient-specific care plans. You’ll give our patients companionship and help with basic medical care, light housekeeping duties, personal care, and hygiene tasks so that they are empowered to live happily at home. Responsibilities: • Help patients accomplish personal care tasks like scheduling appointments, calling doctors’ offices, and coordinating Medicare and other insurance details • Stay on top of light housekeeping tasks including but not limited to laundry, bed making, vacuuming, etc. • Implement care plans as directed by the case manager, and keep updated and accurate care records • Maintain psychological and emotional support for patients and family members - provide quality companionship • Administer basic medical tasks like taking vital signs, measuring glucose and ketone levels, replacing bandages and dressings, and ensuring the proper use of medical devices • Administer Medications: Dispense medications as directed by the Registered Nurse (RN), maintaining an annual error rate of no more than one mistake. • Resident Response: Attend to resident call lights within 10 minutes to ensure responsive and attentive care. • Light Housekeeping: Perform regular light housekeeping in resident rooms at least once weekly to promote a clean and comfortable living space. • Service Charting: Accurately document all services provided without missing tasks, ensuring compliance with company standards. Qualifications: • Possess a valid state driver’s license, current auto insurance, and reliable transportation • Empathetic, compassionate, and kind personality with excellent organization and communication skills are desired • Some college preferred, but a high school diploma or GED is required • Current CPR certification and first aid training is needed and you must be able to pass a full employment background check, drug test, and physical • 1-3 years of experience as an HHA/ home health aide, personal caregiver, nursing assistant, or personal care aid for a home health agency, at patient homes or care facilities settings is needed • Extreme Ownership: Demonstrates accountability, ownership of outcomes, and proactively removes obstacles. • Resourcefulness: Finds effective solutions even with limited resources and independently seeks knowledge. • Passion for Purpose (Big Why): Possesses a strong, genuine dedication to serving and caring for vulnerable adults. • Commitment to Growth: Actively pursues self-improvement and team betterment each day. • Humility: Embraces a solutions-focused attitude, welcoming feedback and putting egos aside for the best outcome. • Reliability: Shows up on time, prepared, and ready to deliver dependable support. • Gratitude: Maintains a positive outlook, seeking and appreciating the good in all situations.
