Are you an experienced Registered Nurse (RN) with a passion for providing exceptional care in a home health setting? Our fast-growing, dynamic home health agency is seeking an experienced RN Case Manager to join our team. This position offers a chance to be part of an exciting, expanding company with opportunities for advancement, including the possibility of a management position in the near future.

Key Responsibilities:

Manage and coordinate care for patients in their homes, ensuring the delivery of high-quality, compassionate care.

Develop, implement, and evaluate individualized care plans.

Work closely with physicians, families, and the interdisciplinary team to ensure the best patient outcomes.

Provide clinical assessments, monitoring, and education to patients and their families.

Supervise home health aides and support staff as needed.

Ensure compliance with all regulatory requirements and maintain accurate documentation.

Qualifications:

Active and unrestricted RN license.

Minimum of 2 years’ experience in home health or case management.

Strong clinical assessment, critical thinking, and communication skills.

Ability to work independently, with excellent time management skills.

BLS/CPR certification required (other certifications preferred).

Management experience a plus, but not required.
