We are seeking a dedicated and compassionate Case Manager to join our home health team. The ideal candidate will play a crucial role in coordinating patient care, and ensuring that individuals receive the necessary services and support throughout their treatment journey. This position requires strong communication, collaboration with other healthcare professionals, and consistent follow up with nurses and patients.
PREVIOUS HOME HEALTH OR HEALTHCARE FACILITY EXPERIENCE REQUIRED Duties
Developing care plans With input from the patient and family, home health case managers create a plan of care that addresses the patient's needs. They present the plan to the patient's physician and health plan for approval.
Home health case managers coordinate the efforts of the patient's healthcare team, which may include physical therapists, occupational therapists, home health aides, and other providers. They also supervise visiting nurses and home health aides.
Home health case managers discuss treatment plans and outcomes with patients and their families and educate them.
Home health case managers regularly report on the patient's condition, progress, and care to the patient's physician.
Home health case managers communicate with the patient's health insurance company.
Home health case managers monitor the patient's medical, wellness, and specialist treatment needs.
Home health case managers prepare medical documentation, such as progress updates, orders, medication order changes, and care plan changes. They also prepare visit and shift reports.
Home health case managers ensure that all legal and ethical standards are met.
Experience
A background in Home Health Services is preferred.
Familiarity with discharge planning processes and utilization management practices is essential.
Intake or general healthcare experience.
Excellent interpersonal skills with the ability to build rapport with patients from diverse backgrounds.
