We are accepting applications for a Full-Time Home Health Nurse Coordinator (RN) position, Monday through Friday, from 8:00 AM to 4:30 PM. In this role, you will provide and coordinate healthcare services for independent residents admitted to RiverMead Home Health Care. Responsibilities include conducting assessments, providing health education, and referring to appropriate agencies. You will also monitor job performance, complete evaluations, and offer counseling to home health staff.

• ******Graduate of an accredited school of nursing**
• ** Valid nursing license to practice in the State of New Hampshire.**
• ** Minimum of three years of nursing experience is required.**
• ** CPR certification required**
