    Care Net Home Health is a sister company to our Angel Care Hospice & Palliative agency in Arcadia, CA. Main responsibilities pertaining to this position is the coordination of all aspects of the patient intake process. This includes collaborating with members of the intake team, establishing and maintaining positive relationships with patients and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes.

    A successful candidate should be professional, organized, well spoken, prior home health experience and dedicated to patient care. They should have strong home health/hospice knowledge of eligibility, operations and demonstrate exceptional ability to communicate in an interdisciplinary team framework.     Responsibilities:

    Directs all daily patient referral and intake operations including providing oversight of the establishment and implementation of intake policies.

    Conduct comprehensive assessments of patients' medical history, current condition, and treatment goals.

    Coordinate and monitor patient care, ensuring that all necessary medical services and resources are provided.

    Monitor and evaluate the effectiveness of treatment plans and make adjustments as necessary.

    Ensures compliance with all state, federal, and CHAP referral/intake regulatory requirements.

    Contributes to the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.

    Establishes and maintains positive working relationships with current and potential referral sources.

    Ensures seamless transition of patients to home care by providing coordination of patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.

    Ensures maximum third-party reimbursement through oversight of insurance verification and authorization processes.

    Gathers and reports referral statistics including key customer referral trends.

    Contributes to strategic planning including identifying opportunities for additional or improved services to meet unmet customer needs.

    Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources.
