What You’ll Do

• Deliver quality home-based care to patients across the Greater Phoenix area

• Follow care plans and coordinate with RNs, therapists, and physicians

• Educate and support patients and their families

• Document care using WellSky/Kinnser (preferred)

• Provide compassionate, efficient, and skilled nursing care

What You’ll Need

• Active LPN License (Arizona State Board of Nursing)

• Home Health Experience (required)

• Familiarity with WellSky/Kinnser (preferred but not required)

• Current CPR certification

• Valid Arizona driver’s license , insured personal vehicle

• TB clearance (or willingness to obtain)

• Graduate of an accredited LPN program
