    Licensed Practical Nurse (LPN) PRN Position Region A: Santa Rosa County: Pace, Milton, Gulf Breeze, Navarre Escambia County: Cantonment, Pensacola

    Job Summary

    We are seeking a Licensed Practical Nurse to join our team. In this role, you will primarily be following the plan of care for created by the RN for individual patients. You are a Licensed Practical Nurse (LPN) who operates with great attention to detail and a big heart. You take pride in carrying out personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as great communication skills, self-motivation, and a strong ability to solve problems.

    Responsibilities

    Coordinate with other colleagues to assess, evaluate, and follow the patient care plans created by the RN in a community based setting (home health) in the following locations:

    Santa Rosa County: Pace, Milton, Gulf Breeze, Navarre Escambia County: Cantonment, Pensacola

    Assess and monitor patients for any abnormalities and/or changes in status

    Document any changes in patient conditions and report on changes

    Maintain accurate records of patient care and discuss observations with supervisor

    Productivity Schedule: Minimum of 10pts per week 1 visit = 1 point

    Qualifications

    Currently licensed Licensed Practical Nurse (LPN) in Florida

    Previous experience as a Home Health LPN is preferred

    Excellent interpersonal and client care skills

    Valid driver’s license, reliable transportation and active auto insurance

    BLS certification required

    IV Therapy Certification is preferred, not required
