About the Role

As a Medical Social Worker , you will play a vital role in patient care by:

• Implementing individualized care plans
• Conducting assessments and providing patient progress reports
• Supporting patients and families with self-care strategies
• Coordinating home health services

Qualifications

• Valid Medical Social Worker license in Arizona (in good standing)
• Master’s degree in Social Work
• Minimum of one year of supervised post-certification experience in a healthcare setting
• Home health experience preferred
