We are seeking a skilled and compassionate Part-Time/Per Diem Occupational Therapist (OT) to join our home health care team. In this role, you will help patients regain their independence by improving their ability to perform daily living activities in the comfort of their homes. The ideal candidate will have a passion for helping individuals recover from illness, injury, or surgery while providing tailored care plans that meet each patient’s unique needs.
Key Responsibilities:
Conduct thorough patient evaluations to assess physical, cognitive, and functional abilities.
Develop and implement individualized treatment plans aimed at improving daily living skills, fine motor skills, and overall functional independence.
Provide hands-on therapy, including exercises and activities designed to improve strength, coordination, and mobility.
Educate patients and their families on techniques and strategies to enhance independence and promote safety within the home.
Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated patient care.
Document patient progress, treatments, and changes in condition in the electronic medical record (EMR) system.
Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
Adhere to agency policies, procedures, and state/federal regulations to ensure high-quality care and compliance.
Qualifications:
Active and unrestricted Occupational Therapist (OT) license in Florida.
Minimum of 1 year of clinical experience, preferably in home health, rehabilitation, or a related field.
Strong understanding of home health care practices, rehabilitation principles, and state regulations.
Proficiency in electronic medical record (EMR) systems.
Excellent communication and interpersonal skills.
Ability to work independently and manage multiple patients effectively.
Compassionate, patient-focused, and detail-oriented.
Valid driver’s license and reliable transportation.
