Home Health Occupational Therapist

We are a Medicare-certified home health and hospice agency that is seeking an experienced and caring full-time Home Health Occupational Therapist for our Home Health division in Grants Pass and Medford, Oregon.

Compensation Package:

• Up to $60 Per Hour (Based on Experience)

Requirements:

• Clear and active Occupational Therapist license in Oregon
• New graduates considered!
• Community/home health experience is preferred
• Possesses and maintains current CPR certification

Responsibilities:

• Monday through Friday 7:30AM to 4:30PM
• Part Time Options!!
• Provide care to patients in the privacy of their home setting
• Using HCHB
• Working through admissions and routine visits to develop and carry out care plans
• Participating in interdisciplinary meetings to collaborate on approaches to patient care
• One-on-one time with your patients
