    We are seeking a Home Health Occupational Therapist to join our rapidly growing team.

    POSITION SUMMARY: The Home Health Occupational Therapist will provide therapy services to patients in their homes, helping them regain their ability to perform daily activities and enhance their quality of life. This role involves assessing patients' needs, developing personalized treatment plans, and collaborating with caregivers and healthcare professionals to ensure optimal outcomes.

    ESSENTIAL FUNCTIONS:

    Conduct comprehensive assessments of patients’ functional abilities and develop individualized treatment plans.

    Provide hands-on therapy to improve mobility, coordination, and daily living skills.

    Educate patients and caregivers on adaptive techniques and equipment.

    Monitor progress and adjust treatment plans as necessary.

    Collaborate with interdisciplinary team members to ensure holistic patient care.

    Maintain accurate and timely documentation in compliance with regulatory and company standards.

    Ensure patient safety and adherence to prescribed therapy programs.

    QUALIFICATIONS:.

    Current and unrestricted California Occupational Therapy license.

    CPR/BLS certification.

    Familiarity with state healthcare regulations and compliance standards.

    Expertise in modern occupational therapy techniques and programs.

    Strong interpersonal skills to provide compassionate care for diverse patients.

    Proficiency in treatment assessment and plan development.

    At least 1 year of recent experience in institutional and home health care settings.

    Valid driver’s license with an insured, reliable vehicle.

    Basic computer skills and experience with Electronic Medical Record software (Kinnser preferred).
