    Key Responsibilities:

    • Develop and Implement Treatment Plans: Create personalized physical therapy plans for patients, helping them regain mobility and achieve recovery goals.
    • Patient Education: Educate patients and their families on therapeutic exercises and techniques to promote continued progress after therapy sessions.
    • Collaboration with Healthcare Teams: Work closely with physicians, nurses, and other healthcare professionals to ensure a holistic and coordinated approach to patient care.
    • Document Progress: Maintain accurate records of patient assessments, treatment plans, and progress, ensuring continuity of care.

    Qualifications:

    • Licensed Physical Therapist in the state of Arizona.
    • At least one year of experience as a physical therapist, preferably in a home health setting.
    • Excellent communication and interpersonal skills with the ability to build rapport with patients and collaborate effectively with a care team.
    • Compassionate, patient-centered approach to therapy and care.
    • Must have a reliable vehicle and valid driver's license to travel between patient homes.
