What You'll Do

• Create Individualized Treatment Plans tailored to each patient’s goals

• Lead Hands-On Therapy Sessions to help patients improve mobility, strength, and confidence

• Educate Patients and Families on exercises, home safety, and wellness practices

• Collaborate Across the Care Team to ensure continuity and quality of care

• Document Progress using clear, compliant clinical notes

What We’re Looking For

• Active Arizona Physical Therapist (PT) license

• Home health experience is a plus, but we’ll support the right candidate

• Strong interpersonal skills and a patient-first mindset

• Commitment to empowering people in their healing journey
