POSITION SUMMARY:
Home Health Physical Therapists deliver individualized care to patients in their homes, emphasizing improved mobility, pain management, and enhanced physical function. They evaluate patients' conditions, develop customized treatment plans, and administer therapies to support recovery, safety, and overall well-being in the comfort of a home environment.

ESSENTIAL FUNCTIONS:
Plan and deliver skilled physical therapy treatments for patients with injuries, illnesses, or functional disabilities, aiming to maximize their physical potential.
Evaluate patients' physical strengths and collaborate with physicians to develop personalized treatment plans.
Create and adjust conditioning programs aligned with physicians' treatment plans and patient goals.
Communicate with physicians and clinical team members to refine therapy programs and plans of care.
Provide clear care instructions for Physical Therapy Assistants and Home Health Aides, and monitor their documentation and performance.
Maintain accurate and timely records of patient care, including evaluations, progress reports, and discharge summaries.
Report patients' progress to physicians, care managers, staff, and families.
Perform other responsibilities as directed by physicians or clinical managers.
QUALIFICATIONS:
Current and unrestricted California Physical Therapy license.
CPR/BLS certification.
Familiarity with state healthcare regulations and compliance standards.
Expertise in modern physical therapy techniques and programs.
Strong interpersonal skills to provide compassionate care for diverse patients.
Proficiency in treatment assessment and plan development.
At least 1 year of recent experience in institutional and home health care settings.
Valid driver’s license with an insured, reliable vehicle.
Basic computer skills and experience with Electronic Medical Record software (Kinnser preferred).
