What You’ll Do

• Perform assessments and create personalized care plans

• Educate patients and families about medications and treatment

• Coordinate care with providers and home health teams

• Accurately document patient progress using OASIS

What You’ll Need

• Active Arizona RN license

• Home health experience , including OASIS documentation

• Current CPR/BLS certification (or willingness to obtain)

• Valid driver’s license and reliable transportation

• TB test & fingerprint clearance (we can assist)

• Bilingual in Spanish? That’s a plus!

• Graduate of an accredited RN program
