JOB DESCRIPTION SUMMARY Title/Position: REGISTERED NURSE- PER DIEM COVERAGE AREA: 20-25 miles from Duarte/ El Monte/ Arcadia, Azusa, Los Angeles
*IVIG CERTIFICATION/ EXPERIENCE & PREVIOUS HOME HEALTH EXPERIENCE REQUIRED
SPANISH SPEAKING PREFERRED
Registered Nurse
Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
Regularly re-evaluates patient nursing needs.
Initiates the plan of care and makes necessary revisions as patient status and needs change.
Uses health assessment data to determine nursing diagnosis.
Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
Counsels the patient and family in meeting nursing and related needs.
Provides health care instructions to the patient as appropriate per assessment and care plan.
Identifies discharge planning needs as part of the care plan development and implements before discharge of the patient.
Required qualifications:
Education to meet certification, license or registration requirement. Upon hire: California Registered Nurse License/ California Vocational Nursing License National Provider BLS - American Heart Association 1-year Professional nursing experience ***** PREVIOUS HOME HEALTH EXPERIENCE WITH IVIG INFUSION REQUIRED****
IV and Central lines experience. Experience with wound care including use of negative pressure devices.
Pay Structure: Pay Per Visit
Responsibilities
Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
Administer medications and monitor patients for side effects or reactions
Document any changes in patient conditions and report on changes
Maintain accurate records of patient care and discuss observations with the supervisor
Qualifications
Currently licensed Registered Nurse (RN) in state of employment
Previous experience as a Registered Nurse is preferred
Excellent interpersonal and client care skills
Valid driver’s license
First aid/CPR certification required
