Key Responsibilities:
- Plan and Direct Care: Organize and oversee home care services to meet the specific needs of each patient.
- Conduct Assessments: Perform thorough assessments of patients’ healthcare requirements.
- Create Care Plans: Develop personalized care plans in collaboration with other healthcare professionals.
- Provide Education: Offer counseling and education to patients and their families, ensuring they understand healthcare instructions.
- Communicate Effectively: Document clinical notes and communicate patient conditions to primary physicians and the care team.
What We’re Looking For:
- Licensure: Valid RN License with the Arizona State Board of Nursing.
- Experience: At least one year of experience in home health care or hospice preferred.
- Transportation: Valid Arizona driver’s license and reliable transportation.
- Certifications: Current CPR certification and a negative TB test (we can assist you with this).
- Background Clearance: Satisfactory fingerprint clearance per federal and state regulations.
- Bilingual (Preferred): Conversational Spanish skills are a plus.
Education:
- Associate’s Degree in Nursing or higher education required.
