Your Role:

As a Home Health RN, you’ll be the go-to care expert, guiding patients toward better health, comfort, and independence.

What You’ll Do:

• Conduct in-home assessments and create personalized care plans tailored to each patient’s unique needs
• Administer medications and treatments as directed by physicians, ensuring safety and comfort
• Provide essential wound care and chronic disease management
• Monitor progress and adjust care plans as needed for optimal patient outcomes
• Educate patients and families to support wellness, independence, and confidence
• Supervise LPNs and HHAs to ensure consistent, high-quality care
• Collaborate in interdisciplinary team meetings to keep everyone on the same page about patient progress
• Document care accurately and promptly to maintain a clear, comprehensive patient record

What We’re Looking For:

• Current RN license in Arizona (in good standing)
• At least one year of RN experience —home health, hospice, or critical care experience preferred
• Current CPR certification
• Strong communication and organizational skills
• Ability to work independently and travel to patients’ homes
• Valid driver’s license and reliable transportation
