We are looking for a dedicated and patient-focused Part-Time/Per Diem Speech Therapist (Speech-Language Pathologist) to join our home health care team. As an SLP, you will work with patients in their homes to evaluate, diagnose, and treat communication, speech, and swallowing disorders. Your expertise will help improve the quality of life for individuals recovering from illness, injury, or surgery.
Key Responsibilities:
Conduct thorough assessments to evaluate speech, language, communication, and swallowing disorders.
Develop and implement individualized treatment plans to address speech, language, and swallowing difficulties.
Provide direct therapy and interventions to patients in their homes, focusing on improving communication and swallowing functions.
Educate patients, families, and caregivers on strategies to manage and improve communication and swallowing abilities.
Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care and optimal patient outcomes.
Monitor patient progress and make necessary adjustments to treatment plans to achieve therapy goals.
Document patient evaluations, treatments, and progress in the electronic medical record (EMR) system.
Maintain compliance with agency policies, procedures, and state/federal regulations related to speech therapy in home health.
Qualifications:
Active and unrestricted Speech-Language Pathologist (SLP) license in Florida.
Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA).
Minimum of 1 year of clinical experience, preferably in home health, rehabilitation, or a related field.
Strong understanding of speech-language pathology practices and home health care regulations.
Proficiency in electronic medical record (EMR) systems.
Excellent communication, interpersonal, and organizational skills.
Ability to work independently and manage multiple patients efficiently.
Compassionate and patient-centered approach to care.
Valid driver’s license and reliable transportation.
