    Reports To: Program Administrator

    FLSA Status: Exempt

    Supervises: Staff

    Schedule of Employment: 5 days a week.

    Hours: 8 hours/days (8AM to 4PM)

    Minimum Educational Requirements:

    High school diploma or GED required. Associate degree or higher from an accredited institution of higher learning with a specialization in psychology, mental health and human services, behavioral health, behavioral sciences, social work, human development, special education, counseling, rehabilitation, sociology, nursing or closely related field preferred.

    Minimum Experience Requirements:

    Minimum of 2 years of experience working in the developmental disabilities field preferred.

    Essential Responsibilities:

    · Assist in coaching, support, training and development of assigned Direct Care staff.

    · Facilitate planning and staff meetings as assigned.

    · Communicate all program and staff concerns to Program administrator.

    · Ensures an individual client’s plan is implemented.

    · Model appropriate communication and presentation in the community for client.

    · Provide support and advocate for client as appropriate.

    · Assist clients in skill building and in reaching their goals.

    · Provide crisis and emergency behavior management.

    · Administer medication and prepare proper documentation compliance in accordance to State of ME regulatory guidelines and practices.

    · Work in partnership with individual to access activities and community resources.

    · Ensure health and safety of clients.

    · Ensure client’s nutritional and hygiene needs are met.

    · Provides assistance in personal care and medical needs as defined in individual plan.

    · Provide and model boundaries and limit setting.

    · Provide transportation as defined in plan and approved by Program administrator.

    · Maintain and submit accurate and timely documentation to meet contractual/ licensing standards, policies and procedures. Understand mandated reporting.

    · leadership and teamwork – works well with others in the interest of the individuals we support

    · Attend and participate in mandatory team meetings and required trainings.

    · Participate in the collection and interpretation of data; and apply data to assist in improved practices for service delivery.

    · Team Coordinator are required to work at any program that may have a need and will be assigned and directed to other programs prior to and or during scheduled shifts

    · Record timesheet as directed in When I work.

    · Track and record clients’ goal progress.

    · Assign staff chores, client to work with and community activities based on clients goal.

    · On call as needed.

    · Other duties as assigned.

    Qualifications:

    · High School Diploma or GED required.

    · Minimum of one (2) year of related experience required.

    · Valid State issued Drivers’ License and proof of current insurance required.

    · Maintain DSP/CDS, CPR/FA, behavior intervention training, CRMA and other trainings mandated by Agency policies and State regulations.

    · Proficient personal computer skills including electronic mail, word processing and internet. Ability to learn applicable software required for documentation.

    · Ability to make well‐reasoned decisions, both independently and as part of a team.

    · Ability to follow‐through to meet individual or agency needs.

    · Ability to use discretion in handling confidential information, compliance with HIPAA regulations.

    · Coping skills, such as flexibility in dealing with issues typical to a social service setting.

    · Ability to communicate clearly, in English, (speaking, reading, writing, listening and presentation with individuals and service team members).

    · Deaf Services programs must have the ability to communicate using visual/ gestural communication a/o ASL.

    Other: As an employee of HCSS, the team coordinator is expected to adhere to the policies

    and procedures of HCSS. They must keep client health care information confidential, and in all other ways comply with HCSS policies and procedures implementing the Health Insurance Portability and Accountability Act (otherwise known as HIPAA). Due to the nature of the work, staff at HCSS are required to be trained initially and annually in good health practices, communicable illnesses, and universal precaution procedures. By virtue of the responsibilities of this position. It is reasonably anticipated that the team coordinator may be exposed to blood or other potentially infectious materials. Therefore, they will receive bloodborne pathogens training (including training regarding Hepatitis B) and will be required to get Hepatitis B upon hire or show proof of vaccination. Maintain a current driver’s license and clean driving record as outlined in HCSS’s Driving policy.

    General Sign Off: The employee is expected to adhere to all agency policies and to act as a role model in the adherence to agency policies. This position requires a full attendance at the program. The employee will be subject to an evaluation after 90 days of employment.

    Trainings Required:

    · HCSS Organizational Orientation.

    · Maintain mandatory training and certifications as required.

    Sedentary - While performing the duties of this job, the employee is regularly required to stand; walk; sit and talk or hear. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include Close vision, Distance vision, Peripheral vision, Depth perception and Ability to adjust focus. The employee is frequently required to use hands to finger, handle, or feel and reach with hands and arms.

    · Required to sit for long periods of time.

    · Infrequent light physical effort required

    · Requires driving own vehicle to various sites.
