The Continuous Care (CCN) Hospice RNs primary responsibility is to provide direct patient care in accordance with physician’s orders. The CCN Hospice RN provides continuous care visits under the supervision and direction of the RN Case Manager and in accordance with the plan of care. We are looking for two individuals who will rotate 3 shifts one week and four shifts the following week, with a every other weekend rotation. This is a salaried position from 6 PM to 6 AM, and you will be compensated even if there is no overnight case.
Workflow: • Provide direct patient care as assigned. Report all pertinent findings to the RN Case Manager, physician, and Clinical Manager or designee. • Provide palliative nursing care to patients and families, incorporating psychosocial, cultural, physical, spiritual, and biological components and appropriate nursing intervention as identified in the plan of care developed by the IDG. • Demonstrate competent clinical skills in the areas of medication administration and titration, symptom management, and patient/family relationships. • Observe and report signs of adverse reactions and response to treatments to team members and physician. • Consult and collaborate with Hospice IDG to promote quality and continuity of patient/family care. • Document observations/interventions to support higher level of care. • Attend patient deaths; prepare body for transport as needed, educate family on drug disposal methods. • Provide bereavement support to the patient/caregiver family. • Actively participates in the coordination of patient’s hospice care, in accordance with current professional standards and practice, including contributing to patient and family counseling and education. • Communicate effectively and accurately to team members, patients, families and physicians • Identify problems and potential problem situations to RNCM and/or Clinical leader as they become evident. • Provides dietary counseling. • Assist as a back-up or support to other roles as needed. • Is computer literate, enters information into computer in a timely and accurate manner. Uses electronic means of communication effective within agency • Follows all agency policies and practices. • Asks for help when needed. • Participate in hospice’s total quality improvement process as well as hospice in-service training. • Accepts and utilizes supervision constructively in order to enhance the quality of agency documentation and professional self-development
Qualifications:
Graduate of an accredited school of nursing.
Currently licensed as a Registered Nurse in the State of Connecticut.
A minimum of two years of clinical experience
Preferred, minimum one year of hospice or home care experience with a strong knowledge base of community care and associated regulations.
Ability to handle several situations simultaneously in a professional, calm, dependable manner.
Ability to prioritize multiple tasks in an organized manner.
Exceptional customer service skills including phone manner and ability to build relationships in a fast-paced environment.
Is computer literate, enters information into computer in a timely and accurate manner. Uses electronic means of communication effectively within agency
