Hospice Case Manager (RN)

A Hospice Case Manager (RN) is needed in Kingston, NY.

Salary and Benefits

• The pay range: $80,000 - $90,000

Responsibilities

• Monday – Friday (8:30 am – 4:30 pm)
• On Call Rotation (3-4 times a year)

• Provide palliative nursing care and symptom control in accordance with physician orders and IDG plan of care.
• To document and enter all interventions of phone and home visits into EMR within 24 hours of the intervention. Medication changes will be entered by the end of the shift.
• Teach, supervise and counsel patients/families/care givers regarding physical care, observing return demonstration of procedures as appropriate.

Requirements

• Clear and active RN license in the state of New York.
• Hospice, Home Health, ICU, CCU, Oncology, End-of-Life Care or OB experience preferred.
