The Hospice Clinical Leader / Hospice Clinical Educator is a hybrid role designed to offer leadership, clinical support, and education to both new and existing hospice staff. This dynamic role combines office-based tasks such as overseeing clinical operations, staff development, and policy updates, with field-based responsibilities that include mentoring clinicians during real-time patient care. You will play a crucial role in ensuring that hospice staff adhere to best practices, regulatory standards, and the organization’s mission of delivering compassionate, high-quality care.
Key Responsibilities:
Provide daily clinical leadership, guidance, and support to the interdisciplinary hospice team (IDT) to ensure compliance with organizational policies, state/federal regulations, and best practices.
Supervise and mentor clinicians, including RNs, LPNs, aides, and other care team members.
Oversee case management to ensure patients’ needs are met, and care plans are effectively executed.
Collaborate with the Hospice Director to monitor clinical outcomes, quality metrics, and patient satisfaction.
Lead team meetings to review cases, discuss clinical challenges, and provide feedback.
Develop and deliver orientation and training programs for new hires, focusing on hospice protocols, policies, and clinical practices.
Provide hands-on mentorship to clinicians, particularly new hires, through field-based support to ensure high standards of care.
Perform joint patient visits with clinicians, offering real-time feedback, support, and education on complex cases.
Serve as a clinical resource by offering guidance on patient care planning, medication reconciliation, symptom management, and family support.
Provide ongoing education to current staff, particularly in areas like symptom management, end-of-life care, pain management, and family communication.
Experience & Qualifications:
Bachelor of Science in Nursing (BSN) from an accredited program required.
Minimum of 2 years of hospice experience, with 5 years of nursing experience.
Current Registered Nurse (RN) license in the state of Connecticut.
Certification in Hospice and Palliative Care preferred (required within 2 years of hire).
Ability to identify needs in workflow processes and implement improvements.
Knowledgeable and compliant with agency and regulatory procedures, including but not limited to Corporate Compliance, HIPAA, Medicare Conditions of Participation, State of CT Licensure, and CHAP Standards.
