Description: Clinical Liaison, LVN/RN
Job Details Job Type Full-time San Antonio, TX
Description The Clinical Liaison is responsible for acting as a partner between the community and the agency by seeking out referral opportunities and relationships, ensuring collaboration of care, coordinating discharge planning, and maintaining excellent communication. They will assist in developing relationships with physicians and clinical staff, meet with patients and families to educate them on hospice services, communicate with intake on the next steps, and ensure patients get the right service at the right time. The Clinical Liaison provides nursing care to patients who require such during idle time.
Requirements QUALIFIED CANDIDATE WILL POSSESS:
Unrestricted LVN/RN license in the State of Texas
Canidates with established community relationships (preferred)
Diploma must be from an approved school of professional nursing
Excellent communication and organizational skills
Minimum experience of 2 years hospice, medical surgical, critical care or oncology nursing
Hospice experience preferred
Must maintain current CPR certification
Must hold and maintain a valid Texas driver’s license and automobile insurance
TB status must be current
