Crest Palliative care, a growing hospice program, has an exciting opportunities for a Hospice Coordinator.
The Hospice Coordinator is a key member of our operational team. This position is responsible for ensuring our patients receive 5 star patient care and customer service. The candidate must work well in a high pressure environment and comfortable working with a variety of healthcare professionals. Prior hospice experience required. LVN preferred but not required.
Job duties include but are not limited to scheduling, ordering medical supplies, medical records, chart review and intake support.
We serve San Antonio and the Hill Country. Our patients are in their Homes, Personal Care Homes, Assisted Living and Long Term Care facilities. .
