Job Description: Hospice Home Health Aide
We are seeking a compassionate and dedicated Hospice Home Health Aide to join our Dekalb County team. As a Hospice Home Health Aide, you will conduct daily patient home visits and provide essential personal care and support to patients and families facing end-of-life stages. Your primary responsibility will be to ensure the physical and emotional well-being of our patients while respecting their dignity and individual choices. This role requires a strong sense of empathy, excellent communication skills, attention to detail, and the ability to work effectively as part of a multidisciplinary team.
Responsibilities:
1. Provide personal care assistance to patients, including bathing, grooming, toileting, and oral hygiene, ensuring their comfort and maintaining their hygiene needs.
2. Assist patients with mobility, transfers, and ambulation, while ensuring their safety and reducing any risk of falls or injuries.
3. Support patients in following their prescribed medication and treatment plans, under the supervision of a registered nurse or healthcare professional.
4. Monitor and report any changes in the patient's physical, mental, or emotional condition to the appropriate healthcare professionals.
5. Assist with light housekeeping duties, maintaining a clean and safe environment for the patient, while respecting their personal space and belongings.
6. Offer emotional support and companionship to the patient and their family, actively listening to their concerns, fears, or needs.
7. Provide respite care to patients' families, allowing them time to rest or attend to their personal needs, while ensuring the patient's well-being.
8. Aid in the preparation and serving of meals for patients, adhering to any dietary restrictions or preferences.
9. Collaborate effectively with the interdisciplinary team, including nurses, social workers, and chaplains, to develop and implement personalized care plans.
10. Participate in ongoing training and continuing education programs to enhance knowledge and skills in end-of-life care, palliative care, and hospice services.
11. Adhere to all relevant healthcare regulations, policies, and procedures while maintaining patient confidentiality at all times.
Requirements:
1. High school diploma or GED equivalent.
2. Certification as a Certified Nursing Assistant (CNA), Home Health Aide (HHA), or any relevant healthcare certification is highly preferred.
3. Previous experience in a hospice or palliative care setting is desirable.
4. Demonstrated understanding and knowledge of end-of-life care principles, including pain management, comfort care, and emotional support.
5. Strong communication and interpersonal skills, with the ability to build trust and establish rapport with patients and their families.
6. Compassionate and empathetic nature, with a patient-centered approach to care.
7. Ability to adapt and respond effectively to the changing needs of patients in a sensitive and empathetic manner.
8. Excellent organizational and time management skills, with the ability to prioritize tasks and work independently while adhering to established care plans.
9. Proficiency in basic computer skills and the ability to maintain accurate documentation of patient care activities.
10. Physical stamina and ability to perform lifting, pushing, and pulling tasks while assisting patients with mobility.
11. Current CPR certification is preferred.
