    Key Responsibilities:

    • Manage the intake process for new hospice patients, ensuring all necessary paperwork, documentation, and patient information are completed accurately.
    • Collaborate with interdisciplinary teams to schedule initial patient assessments and visits.
    • Communicate with physicians, families, and patients to coordinate smooth transitions into hospice care.
    • Maintain patient records and ensure all data is entered into our system in a timely and accurate manner.
    • Provide ongoing support and customer service to patients and their families throughout the intake process.

    Qualifications:

    • Experience in a hospice is required.
    • Strong organizational and multitasking skills.
    • Excellent communication and interpersonal skills.
    • Ability to work independently as well as part of a team.
    • A compassionate and patient-centered approach to care.
