Job Description: Hospice Liaison - Community Sales Representative

Position Overview: We are seeking dynamic and motivated Hospice Liaisons for our community sales team. The ideal candidates will be responsible for fostering relationships and driving admissions within their assigned territories. As representatives of the oldest nonprofit hospice organization in Georgia, you will play a critical role in expanding our reach and ensuring patients receive compassionate care.
Key Responsibilities:
- Develop and maintain relationships with healthcare providers, community organizations, and other referral sources.
- Educate the community about our hospice services and the benefits of private pay hospice care.
- Utilize CRM software (Trella) to manage and track account activities.
- Meet and exceed monthly admission goals, starting at 10 and aiming for 15-20 starts of care per month.
- Strategically manage 30-60 accounts within your territory, using a smart approach to account management.
- Collaborate closely with internal teams to ensure seamless patient transitions and optimal care.
- Report directly to the COO, maintaining open lines of communication and feedback.
Qualifications:
- Proven experience in hospice care or a related healthcare field.
- Ability to hit the ground running and achieve immediate results.
- Strong sales and relationship-building skills.
- Experience with private pay hospice services is a plus.
- Established book of business is highly preferred for experienced candidates.
Compensation:
- Base Salary *Experienced candidates with a book of business: $70,000 - $110,000
- Commission Structure:
- $850 for 10 admissions
- 400 per after 10
- $2,400 for 16 admissions
- $4,000 for 20 starts of care
- Mileage Reimbursement: $0.65 per mile
- PTO: 17 days, plus 3 flexible holidays and 4 mental health days
